Care Coordination Services

Care Coordination is a part of our Certified Community Behavioral Health Clinic (CCBHC) which acts to expand an array of mental health and addiction services in community based settings. Care coordinators work between Cascade Community Healthcare (CCH) and other healthcare systems and community partners such as primary care physicians, transportation, housing, food and financial assistance, employment, and Veterans services. Also, as a part of care coordination staff act as Washington State Navigators to assist clients in getting setup with state insurance.

 

Another function as a part of this team, under the Crimes Victims grant, involves assisting survivors of abuse and sexual assault to receive treatment and support while coordinating care with community partners.
