Community Support

Following your Intake Assessment, you may be assigned a Case Manager in our Adult Services Program.

This program offers case management services that provides goal-oriented, individualized supports that focus on improved self-sufficiency.

Case Managers will work to coordinate client care needs, explore community supports and activities, and assist clients in meeting their treatment plan goals.
